We are pleased to announce that, following consultation with medical professionals in Wales, CoramBAAF have published Welsh language electronic editions of some of their children’s health forms and these are now available for forms licence holders.

The Consent Form, and Forms IHA-C, IHA-YP, RHA-C, and RHA-YP (initial and review health assessments for children and young people) are now available electronically.  There is no extra charge for these – they are covered by your license for the children’s health forms.

We will keep you informed of further Welsh translations as they are published, as well as other new  and revised forms.
